COMMUNITY HEALTH WORKERS STROKE TRAINING

Community Health Workers (CHW) are an integral part of Post Hospital care. There are programs that have incorporated their role for diabetes management but to date in Florida, there has not been a formal role in the management of post stroke care patients.

This course is designed for the post stroke transition of care, and would be of benefit to community health workers, community paramedics, patient navigators or social workers. Raising Stroke Awareness is integral in our fight against stroke. By participating in this program, you are actively raising stroke awareness in your community.

The Program is led by the Florida Stroke Registry Executive community and is proudly supported by University of Miami faculty who are recognized experts in their field. Dr. Gillian Gordon Perue, Assistant Professor of Neurology and Stroke serves as the Course Director.
